Multi-Sector General Permit Application Forms

Applying for the Multi-Sector General Permit (MSGP) involves a series of steps.  Once you have determined your facility’s SIC (Standard Industrial Classification) code, the rest of the process is fairly straight forward.  If you have questions about how to obtain your SIC code or how to determine if your facility needs coverage under the MSGP, click here: Frequently Asked Questions

If you determine that your facility needs coverage under the MSGP, you may apply online using the eDEC electronic application site or you may print and mail an application form located below.

Both the No Exposure Conditional Exclusion and the Notice of Intent (NOI) may be completed online.  To use the online system you will need a valid email address and a valid driver’s license.

No Exposure Form (pdf, 520 kb) – If your facility qualifies for the No Exposure Conditional Exclusion, please use this form.  You may fill in the form on your computer and save it using Adobe Acrobat.

NOI Form (pdf, 188 kb) – Existing facilities that do not qualify for No Exposure must submit an NOI form immediately.  New facilities must submit an NOI form 90 days prior to commencing activity at the site.  You may fill in the form on your computer and save it using Adobe Acrobat. 

Notice of Termination Form (pdf, 92 kb) - If your facility has a new owner or operator, or if operations have ceased at the facility and there are no longer any materials associated

with the industrial activity exposed to stormwater, please download, fill out, and submit an NOT form.

Discharge Monitoring Report Form (pdf, 192 kb) - Discharge monitoring forms needed for compliance with the Benchmark, Effluent and Impaired waters monitoring requirements
